Submit your Child Online Intake Form

Fill out our simple online intake form to share your needs for autism assessment and services.

A member of our team will contact you to answer your questions and discuss how we can support your family.

Complete your Intake Packet

You will receive an email with detailed instructions on completing the additional intake forms needed for your evaluation.

To help us better understand and support your needs, please gather the following items:

  • Insurance documentation

  • Any IEP (Individualized Education Plan) or 504 Plan, if applicable

  • Copies of school evaluations related to services or IEPs

  • Any previous clinical evaluations you may have

Clinical Review Call

Within a few days of receiving your completed intake packet, our intake team will carefully review your information and reach out to discuss whether AAC is the best fit for your family’s needs.

During this conversation, we’ll address any questions you have about the assessment process, provide clarity on next steps, and ensure you feel supported every step of the way.
